Abstract

This patient-education resource was developed by the New Hampshire Comprehensive Cancer Collaboration's Shared Decision Making Task Force in 2016. The purpose of the rack card is to educate patients about shared decision making and to encourage them to talk with their health care providers about cancer screenings. This resource was developed with plain language and health literacy elements in mind and is written at an 8th grade reading level. Through the New Hampshire Comprehensive Cancer Collaboration, healthcare facilities and other organizations are able to co-brand these rack cards before printing and providing to their patients.
